After completing the internship, students are required to submit Form 12-2, Form 15 with internship feedback and photos, Form 4 Off-campus Internship Record, Form 5 Visit Record, email communication records with the supervising professor, Form 10 Off-campus Internship Evaluation and Certification, Form 11 Supervising Professor’s Evaluation, and complete the Student Off-campus Internship Questionnaire. Additionally, please have the company supervisor fill out the Employer Questionnaire.)
